With regards to treatment, insurance coverage and payment for services performed at the Chronic Headache and Migraine Institute:
The need for treating varies for each patient. The physician will determine which diagnostic tests or procedures are needed based on patient symptoms.
Procedures performed at the Institute will be billed by the physician billing services. All diagnostic tests will be billed by the facility performing the exam.
You may need a physician referral if you are a member of a Health Maintenance Organization (HMO), an Exclusive Provider Organization (EPO) or a Point of Service (POS) plan.
Refer to your insurance card or call your insurance carrier to determine whether you need a referral to be evaluated by the Migraine Institute physician.
Your appointment will be rescheduled if the referral is not obtained for the date of your appointment, as failure to follow your insurance provider's process may result in denial claims and fees for services rendered will become the responsibility of the patient.
