To obtain medical records for St. John Detroit Riverview Hospital patients:
Print and complete the Release of Medical Records Authorization form or request a form by contacting the Health Information Department at the location below. When you complete the form, be sure to specify the dates of service and the type of information needed.

There is a $.20 (20 cents) per page charge for copies of records longer than 25 pages. To determine size of your records, you may call St. John Hospital and Medical Center Health Information Management department at 313-343-3781. Check or money orders should be made payable to RMS-Services USA.
Records may be mailed to the patient or may be picked up in person after Release of Medical Records Authorization form has been received and processed. The Health Information Management department will notify you when records are available to pick up. Those picking up records will be asked to show photo identification.
