St. John Health Chronic Headache and Migraine Institute

Important News for Migraine Institute Patients

Due to a change in clinical leadership, effective June 30, 2007, St. John Health's Chronic Headache and Migraine Institute has stopped accepting appointments, for an unspecified period of time. We are actively recruiting a physician specializing in headache management who will continue to provide patients with quality migraine headache treatment.  This process will take several months.

The information below can help you during this period.

Obtaining Medical Records

Copies of medical record will be made available to patients or their physician upon request. Patients will need to complete an Authorization for Release of Medical Records  form with the necessary information.

  • Document your complete name and address in the indicated area of the form if the records are to be mailed to you.
  • Document the complete name and address of the physician in the indicated area if the records are to be sent to a physician.
  • Return the form in the enclosed return envelope, or fax the form to 248-967-7991.
  • Please allow one to two weeks for records processing.

Note: Some patient records are too large to be sent by mail.  In these cases, patients will be notified if it is necessary to personally retrieve records from our office. We will have very limited office hours during this transition phase.  Please call in advance to determine staff availability.

Treatment and Pain Management Resources

As always, the staff of St. John Macomb-Oakland Hospital, Oakland Center, Emergency Room will be able to assist patients who need treatment for a headache that does not respond to prescribed pain medication.  Your family physician, or an Emergency Room close to home or workplace would be an alternate location for treatment.

We truly apologize for any inconvenience associated with these changes.  For the past four years, it has been our mission to provide you with quality medical treatment for your headaches, and it is our intention to resume this practice as soon as possible. Please be advised that we will notify you by phone when we are able to reschedule appointments.
