Providence Radiology Services - General Information

Bring to your appointment
- Written doctors order and referral or authorization if required - some insurance companies require pre-authorization, if you present for your test/procedure without pre-authorization you may need to be rescheduled
- Insurance card
- Personal identification (drivers license, state ID, etc.)

Locations
We have four locations to serve you: Southfield, Novi, Farmington Hills, Livonia. Please call our toll free scheduling number (1-866-228-9729) to schedule an appointment at any location.

Results
Please contact your physician's office for test results.

Payment
Any insurance co-pays are expected at time of service. We will also request payment for any outstanding balance you may have. We accept check, cash, Visa and MasterCard.

Southfield Breast Center	Monday-Friday 7:30 am-5:30 pm	The Breast Center is located in the PMB (Professional Medical Building) First Floor

Southfield Outpatient Imaging Center	Monday-Friday 8:30 am-4:30 pm	The Outpatient Imaging Center is located in the PMB (Professional Medical Building) Lower Level

Novi	Monday-Friday 7:00 am-8:00 pm	Register at Front Receptionist Desk (Grand River entrance);
	Note: MRI is open later, register directly with MRI
