Introduction
The Hematology Oncology Fellowship at Providence Hospital is a three-year program, which is designed to provide broad clinical experience in the diagnosis and management of solid and hematological tumors along with expertise in blood banking, coagulation, hematopathology, histopathology, bone marrow transplantation, and benign hematology.  The program is organized to include participation and design the implementation of basic and clinical research. 
 
Goals
The program has the participation of experienced clinical faculty, basic science and adjunct faculty including pharmacology, social work, hospice care, case managers, data managers, and clinical research managers and nursing.  This program is geared towards developing expertise in delivery of compassionate care with cost containment and pharmacoeconomics and pharmacogenomics. The fellows will have the responsibility of writing medical orders under the supervision of teaching faculty and following the patients in the outpatient clinic.  End of life issues are integrated within the teaching program. The final goal of this program is a successful board certification in hematology/oncology. 

Objectives


I.   RISK, CAUSE AND EFFECTS OF CANCER:  

 

1.  Include in this are molecular biology with an emphasis on oncogenes
2.  Carcinogens
3.  Genetic counseling and predisposition with special emphasis on breast cancer, malignant
     melanomas, colon cancer, and hereditary conditions, predisposition to malignancies
4.  Screening of programs for cancer
 


II.   SCREENING PROGRAMS FOR CANCER:

 

1.   Skills acquisition
2.   The fellows should be able to identify and manage hematology oncology emergencies
3.   Manage complications of treatments such as chemotherapy, radiation and surgical
      complications of malignancies 
4.   Demonstration of knowledge of the paraneoplastic syndrome
5.   Pain Management
6.   Management of End-of-Life Issues
7.   Use of antiemetics
8.   Management of febrile neutropenia and appropriate cytofine therapy and neutropenia 
9.   Appropriate use of use of chemotherapy
10. Management of clotting disorders 


III.   KNOWLEDGE AND INDICATION FOR PERFORMANCE AND INTERPRETATION (of the following):

 

1.   Bone marrow aspiration biopsy
2.   Blood Banking
3.   Indications for stem cell transplant
4.   Thoracentesis
5.   Lumbar Puncture
6.   Tumor markers
7.   Growth factors
8.   Cytogenetics
9.   Molecular Biology 


IV.  DIAGNOSIS STAGING AND TREATMENT (of the following)

 

1.   Coagulopathies including hemophilias
2.   Management of anemias including hemolytic anemia
3.   Management of malignancies such as cancer of the breast, ovary, uterus, cervix, lung,
      etc.
4.   Hematologic malignancies such as leukemias, lymphomas, and myeloproliferative,
      myelodysplastic syndromes
5.   Genitourinary malignancies, specifically prostate, bladder, kidney
6.   Testicular malignancies
7.   Cancer of the head and neck, and naso-pharynx
8.   Endocrine tumors, specifically thyroid, adrenal, pituitary
9.   Cancer of the aerodigestive tract, more specifically in lung, and esophagus 
10. Gastrointestinal malignancies, primary cancers of the liver and metastatic cancer of the
      liver, stomach, colon, anus
11. Tumor of neuroendocrine origin, such as small cell, malignant melanoma, and islet cell
      tumors


V.  PALLIATIVE CARE

 

1.   Blood transfusions
2.   Chemotherapy
3.   Radiation Therapy
4.   Biotherapy
5.   End-of-life issues and palliative care

These objectives will provide the fellow in Hematology/Oncology the foundation to gain the expertise in the diagnosis and management of solid tumors along with those hematologic malignancies.  They also will help the fellow develop expertise in benign hematology, specifically blood banking, coagulopathies, and hemochromatosis.  This will also allow the participation in clinical trials.


Research
The fellow will work in the laboratory at Providence Hospital or an established agreement at a university program and will be supervised by a Ph.D.  The fellows will be expected to present abstracts and papers either through the American College of Physicians, or the American Society of Hematology, or the American Society of Clinical Oncology.  The fellows will be encouraged to participate in cooperative groups, either through the Southwest Oncology Group or the National Surgical Adjuvant Breast and Bowel Project.

The fellows will have opportunities for participation in multispeciality tumor boards, which meets generally weekly with either the General Tumor Board, Leukemia Lymphoma, Breast Cancer, Lung Cancer, GYN, Liver and Genitourinary.  The fellows also presents during their second year rotations at the University of Michigan. 

Rotations at the University of Michigan will include bone marrow transplantation, clinical pharmacology, along with the attendance of  multi-subspecialty clinics.  The third year will encompasses basic research, while attending lymphoma clinics, blood banking, hemopathology, bone marrow transplantation, management of the acute leukemia and genetic counseling along with consultation in benign hematology.
